and Insurance Policies
Payment
is expected at time of service. Rates are provided
upon request and are posted in our office handouts.
Cancellation
Policy: For the courtesy of others, please provide
48 hours notice if you need to cancel an appointment. There
is a high demand for our services and every missed appointment
is a missed opportunity for someone else who want to get
in to see us. Therefore, you will be charged for missed
appointments with no prior notice of cancellation within
48 hours. We consider missing appointments without notice
a siginificant issue, in that it deprives others from the
opportunity of getting treatment.
Arriving
Late for an Appointment: Your appointment reserves
a specified time for your review of issues by your therapist,
treatment, session documentation, and payment. If you are
late for your appointment, you will be charged the full amount
for scheduled time, regardless of the amount of time you are
in session, due to late arrival. We ask that you respect the
time allotted for your appointment.
Insurance:
We do not directly bill health
insurance companies, however, we will provide you with an
invoice to submit to your insurance company for your own
personal reimbursement. Insurance coverage varies considerably
and our services may not be covered by your insurance. Our
invoices have all the information required by the insurance
companies, including the CPT treatment code.
However, you may be required by your insurance company to
obtain a diagnostic code from your prescribing doctor (only
a doctor, physical therapist, or chiropractor can diagnose,
and therefore, provide diagnostic codes).
Prescriptions:
Obtaining a prescription from you doctor often helps in getting
some level of insurance coverage for your manual therapy work
at our clinic. Doctors, physical therapists, and surgeons
in our local area refer clients specifically to our clinic.
Note that our clinic is not part of any medical network and
therefore, insurance coverage would be considered "outside
network" coverage.
Health
Savings Accounts: We suggest you look into a Health
Savings Accounts (HSA) plan as an alternative to your existing
insurance plan if you plan on using alternative or complementary
health services. Health plans that offer annual individual
reserve accounts for alternative health care are another
option.
Flexible
Accounts for Alternative Medicine: Some companies or
insurance policies provide options to set aside pre-tax money
to pay for alternative medicine therapies, often called "flex
accounts". These accounts offer you more options on where
and how you can spend your health dollars. We suggest you
contact your personnel department or insurance service representative
to find out more information and to explore what is and is
not covered under the flex account. Note that flex accounts
are different from HSA accounts, in that any money that is
not spent within the contract year, is not rolled over into
the next year (as is with an HSA), and you lose that money
(the insurance company keeps the money).
Special
Reports and Documentation Fees: If you need special
reports or treatment documentation sent to other members
of your medical team, for auto accident claims, or legal
purposes, our staff can provide them; however, special
fees may apply. Contact our office for specifics.

accept cash, check, VISA, MC, Discover, and AMX credit cards.
